Student Health Service

St. 四虎影视鈥檚 Student Health Service is a primary care clinic staffed with a team of healthcare professionals to evaluate your health concerns, answer questions and help you be the healthiest version of yourself. Our team is here to support students navigate healthcare concerns, billing issues, immunization compliance and other questions.

Who is Eligible for Service?

  • All full-time undergraduate students taking 9 or more credits
  • All full-time graduate students taking 6 or more credits

All other students may choose to opt-in and pay the Student Health Fee to access Student Health Service.

Available Services

Student Health Service provides a variety of services including immunizations, laboratory tests, routine and sports physicals, general and sexual health and wellness evaluations, illness and injury treatment, referrals, medical supplies and patient education.

Student Health Fee

What is the Student Health Fee?

The Student Health Fee is placed on student accounts Fall and Spring semester automatically for full-time undergraduate students with 9 or more credits. The fee is applied each semester for graduate students taking 6 or more credits enrolled in the following graduate programs listed below.

  • Occupational Therapy
  • Physical Therapy
  • Athletic Training
  • PA Medicine
  • Exercise Physiology

Cost

The fee is $155 per fall and spring semesters. All other students who may want to receive health care in Student Health Service may opt-in for this by contacting studentaccounts@css.edu.

Charges Not Covered by the Student Health Fee

Charges not covered by the Student Health Fee are billed to the student鈥檚 personal/student health insurance. Any charges that are not covered by insurance will be placed on the student鈥檚 account.

  • Nurse Practitioner Office Visits
  • Immunizations
  • Lab tests sent to our reference lab Essentia Health Laboratory including TB tests and STI testing (billed to insurance by Essentia Health)

Student Health Insurance

All St. 四虎影视 students are required to carry health insurance coverage. Domestic students have the option of continuing on an existing parent鈥檚 health insurance policy through age 26. Students who do not have health insurance may purchase policies through a Health Insurance Marketplace.

Domestic Student Health Insurance

In some states, the Health Insurance Marketplace is run by the federal government and in others, it鈥檚 run by the state. MNSure is the Marketplace where Minnesotans can review and enroll in the health insurance coverage that meets their needs.

International Student Health Insurance

All F-1 international students will be automatically enrolled in the insurance plan sponsored by 四虎影视 unless they meet criteria for a waiver. Please see the Accepted International Students page for more information.

Immunization Requirements

Minnesota’s College Immunization Law

requires all students born after 1956 who enroll in a Minnesota college to be immunized against tetanus, diphtheria, measles, mumps and rubella. Minnesota law requires colleges to provide information on transmission, treatment, and prevention of hepatitis A, B, C and the risks of meningococcal disease.

Required Immunizations

St. 四虎影视 requires all new incoming students (first year and transfer) to verify immunization history and complete the online tuberculosis screening form.

Immunization history forms are on file at most high schools and colleges, as well as Primary Care Clinics. Contact these sites directly to request/obtain immunization records. CSS Student Health Service has immunizations available for administration as needed.

Required

  • One Tetanus-Diphtheria-Acellular Pertussis (Tdap) vaccine in the past 10 years
  • One Meningococcal (Meningitis ACWY) vaccine on or after age 16 for students 21 years of age or younger living on campus
  • Two Measles-Mumps-Rubella (MMR) vaccines given on or after age 1 OR positive MMR antibody level (titer)
  • Two Varicella (Chicken Pox) vaccines given on or after age 1 OR positive Varicella antibody level (titer)

Strongly Recommended

  • Up to date COVID-19 Vaccination: up to date means a person has received all recommended COVID-19 vaccines, including any booster dose(s) when eligible
  • Hepatitis B Vaccination series of three (3) doses
  • Meningitis B Vaccination for young adults ages 16 through 23
  • Seasonal Influenza Vaccine

Submit Your Immunization Records

Student Health Service (SHS) requires immunization records for verification. Submit records directly to SHS via mail, email or fax. The deadline for immunization submission is Friday, Sept. 13, 2024.

Tuberculosis Screening Form

You can access your the day after you register for classes. Please fill out the forms to the best of your ability. You only have ONE chance to submit the TB Screening Questionnaire; you will not be able to edit or update it after the initial attempt. Contact SHS to facilitate any update to your TB Screening Questionnaire. Complete this task by Friday, Sept. 13, 2024.

  2. Enter your date of birth (DOB) and click Proceed
  3. Select Requirements & Forms (on the left)
  4. Complete the Tuberculosis (TB) Screening Questionnaire

Immunization Exemption Form

Students electing to request a medical, religious, or conscientious exemption should print, complete, and return the to Student Health Service. A visit with the Nurse Practitioner to discuss and review the exemption may be required in order to be compliant.

Higher Education Vaccine Ambassador

The College’s own Jaci Tourtellott ’22 offers her encouragement to get vaccinated. Jaci was one of four students who participated in the state’s Higher Education Vaccine Ambassador Campaign this fall.

MyHealth

聽is an electronic health portal where students can:

  • Submit health history
  • Make an appointment with a registered nurse
  • Send messages to health care providers
  • Request prescription refills
  • Complete release of information requests
  • View and print immunization records

JED Campus

St. 四虎影视 is collaborating with The Jed Foundation to enhance the mental health work already being done on campus and creates positive, lasting change.

Counseling Services

Confidential, professional clinicians are available to assist St. 四虎影视 students in addressing the feelings and stressors of college life and other critical personal concerns.
